The Pharmacist II in the Nephrology Clinics is responsible for providing direct patient care to patients with nephrology-associated conditions at UC San Diego Health. The pharmacist will conduct virtual and/or in-person visits and work under a Collaborative Practice Agreement to provide comprehensive medication management services, including medication optimization, laboratory monitoring, patient education, medication access support, and chronic disease management. The pharmacist will collaborate with providers, pharmacy staff, and interdisciplinary teams to improve clinical outcomes, address healthcare gaps, and ensure safe and effective use of medications across the continuum of care. The pharmacist in this position may also serve as a preceptor to pharmacy students and residents as assigned and complete other activities at the discretion of managers and supervisors.

Annual Full Pay Range: $200,010 - $238,972 (will be prorated if the appointment percentage is less than 100%)
Hourly Equivalent: $95.79 - $114.45
