Qualifications

• Requires ACPE-accredited College of Pharmacy or FPGEC certification

Responsibilities

• Review prescriptions for accuracy and appropriateness
• Dispense medications for inpatients and outpatients, including unit dose and IV admixture
• Supervise staff and ensure safe, rational medication use
• Document clinical interventions in EPIC and maintain records
• Provide medication counseling and pharmacokinetic/renal dosing monitoring
• Collaborate with providers, nurses, and care teams
• Adhere to organizational competencies and standards of practice
