Job Description/General Duties

This role provides patient-focused pharmaceutical care by reviewing and monitoring medication therapy to ensure safe, effective, and cost-conscious treatment. Responsibilities include medication order verification, dosing adjustments, medication reconciliation, patient education, and participation in multidisciplinary patient care rounds. Minimum Requirements

• Bacherlors degree in Pharmacy
• Licensed Pharm Acist - Start of Pharmacy
• Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)

Preferred Requirements

• Pharmacy Doctorate
• Completion of an accredited post graduate training program (PGY-1 and/or PGY02) residency program
• PALS - Pediatric Advanced Life Support - American Heart Association (AHA)
• −-Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
• Neonatal Resuscitation Program (NRP) - American Academy of Pediatrics (AAP)
