Job Description Summary

The Pharmacist is responsible for clinical review of medication orders for the hospital and serves as a clinical resource to Providers, Nursing staff and other departments. Oversees the preparation and dispensing of medications to inpatients, emergency room and ambulatory clinics. Participates in multi-disciplinary clinical rounds on patient units and provides clinical information to medical staff. Maintains standards set by the department and adheres to policies and procedures of the Pharmacy to meet the standards of Joint Commission. Oversees the Pharmacy Sterile and Non-Sterile Compounding processes. Assists in Quality and Safety improvement initiatives under direction of the Pharmacy Administration Team.

Job Description
ESSENTIAL FUNCTIONS
  • 1. Performs clinical review of medication orders from prescribers throughout the hospital including Emergency room, Critical Care unit, Pediatrics and Medical Surgical units. Intervenes and provides recommendations to providers for drug dosing errors, drug interactions, allergies or drug contraindications. Suggests appropriate or alternate drug therapy to the providers when needed.
  • 2. Responsible for oversight of preparation and distribution of medication orders filled for the in-patient, outpatient and ancillary areas. Responsible for ensuring medications contain the correct labeling and drug components. Responsible for oversight of Sterile, Non-Sterile and Hazardous compounds and ensures adherence to Pharmacy policies.
  • 3. Responsible for reviewing patient medication profiles and monitoring clinical parameters to ensure the proper choice of drug, dosage form, dose, length of therapy and interaction potential of the prescribed drug. Reviews and completes Pharmacy algorithms for antibiotic dosing and Pharmacy drug interchanges for medications not on Formulary.
  • 4. Responsible for training, supervising, educating and checking the work of Pharmacy Technicians and students. Serves as preceptor to pharmacy students when needed.
  • 5. Participates in Multidisciplinary rounds on Nursing Units when needed. Maintains clinical relationship with providers and provides clinical information when needed. Reviews patient clinical records and makes appropriate recommendations when needed.
  • 6. Responsible for maintaining a working knowledge of, and conforming to legal requirements concerning the distribution of drugs, as well as good working knowledge of recent developments in the field of pharmacy. Completes required continuing education as required by law. Completes yearly competencies assigned by the hospital.
  • 7. Assumes \"charge pharmacist\" responsibilities as required, for the coordination of weekend and evening shifts which include supervising the part-time pharmacists and supportive personnel, assigning work and delegating tasks and responsibilities, and for handling problems that arise during the absence or unavailability of the Director of Pharmacy Services.
  • 8. Responsible to function as pharmacy liaison to assigned nursing units for the purpose of handling routine, day-to-day issues. Performs monthly floor inspections as assigned.
  • 9. Responsible for the distribution, control and record keeping of all controlled substances. Participates in weekly inventories of controlled substances when needed.
  • 10. Responsible for conducting, when asked to, in-service education lectures for medical staff, nurses, pharmacists, students and other health care personnel regarding drugs, procedures and policies.
  • 11. Responsible for intradepartmental communication by staff meeting and in-service attendance. Also responsible for knowing content of departmental staff meeting minutes and memos.
  • 12. Safety Awareness – Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment. Participates in safety forums and meetings when asked upon. Participates in quality improvement and safety initiatives when asked.
  • 13. When asked to, is responsible for covering the GERM pager in accordance with Department practices set forth by the AMS service.
JOB REQUIREMENTS
Minimum Education - Preferred

Bachelor's of Science Degree in Pharmacy from an accredited institution preferred.

Advanced degree preferred or enrollment in advanced degree program.

Minimum Work Experience

Minimum of one to three years experience as a Registered Pharmacist in a hospital preferred.

A.S.H.P. accredited residency preferred.
