Job Summary

Provides patient care through direct operational and/or clinical services, ensuring safe and appropriate medication use, drug information, therapy monitoring, and pharmacy services within departmental policies and legal standards. Regular communication with healthcare professionals, patients, and staff is essential. The role involves participation in departmental committees and educational activities. The division of time between operational and clinical duties varies based on staffing and workload.

Essential Functions

  • Adhere to Prisma Health's values: Inspire health. Serve with compassion. Be the difference.
  • Accurately enter medication orders, dispense drugs, administer medications, and perform diagnostic testing and patient care, including specialized populations such as neonates to geriatrics. Aim to minimize turnaround times.
  • Perform pharmacist duties including:
    • Patient Education
    • Patient Care Management
    • Review prescriptions for safety and efficacy
    • Handle prior authorizations and appeals using EMR data and provider contact
    • Educate patients on medications and supportive care
    • Create and monitor individual care plans
    • Develop relationships with providers and staff
    • Refer patients to other healthcare providers or resources as needed
    • Participate in on-call programs where applicable
  • Provide timely and relevant drug information responses.
  • Supervise pharmacy support staff, verify tasks, and possibly serve as lead pharmacist to manage workflow.
  • Complete lab reviews, consults, and document clinical services.
  • Act as liaison between pharmacy and patient care areas to resolve distribution issues and monitor drug storage.
  • Provide training and education for pharmacy technicians, students, and new staff.
  • Represent the department in integrating pharmaceutical care across the patient care continuum.
  • Perform other assigned duties.

Supervisory/Management Responsibilities

  • This is a non-management position reporting to a supervisor, manager, director, or executive.

Minimum Requirements

  • Education: Bachelor's degree from an ACPE-approved college of Pharmacy; PharmD preferred.
  • Experience: None required.

In Lieu Of

  • For those without an active SC Pharmacist license, eligibility includes being a graduate of an ACPE-accredited US pharmacy college with a pending SC intern certificate or reciprocity requirements, subject to board interview.

Required Certifications, Registrations, Licenses

  • Licensed or eligible to be licensed as a Pharmacist within 4 months in the work state's license.

Knowledge, Skills and Abilities

  • Strong knowledge of medical and pharmacy terminology, along with good math, computer, and aseptic skills.
  • Understanding of State BOP/DHEC/DEA regulations.
  • Ability to handle urgent and stressful situations effectively.
