Job description

Under the general supervision of the Director of Pharmacy and the direct supervision of the Pharmacy Management Team, the Geriatric Inpatient Pharmacist is responsible for medication regimen review, drug distribution, and providing clinical pharmacy services within the scope of practice.

Core Competencies
  1. Assesses prescriptions for appropriateness and completeness, documenting changes and clarifications.
  2. Serves as a 'Standards of Behavior' mentor and role model for reporting personnel, as outlined in HR Policy 203.
  3. Supervises and directs ancillary personnel to ensure accurate and timely pharmacy services.
  4. Maintains competencies for compounded sterile and non-sterile products, ensuring proper documentation as required by regulatory bodies.
  5. Provides quality assurance on all medication products, adhering to policies on storage, preparation, labeling, handling, dispensing, and disposal.
  6. Provides drug information to healthcare professionals and the public as needed.
  7. Maintains medication security, complying with the 'Medication Security' policy, and demonstrates appropriate handling of controlled substances according to policy.
  8. Completes annual clinical competencies promptly.
  9. Delivers healthcare services across all patient age groups, including Neonates, Pediatrics, Adolescents, Adults, and Geriatrics.
  10. Responds to Emergency Code pages and prepares medications accordingly.
Department-Specific Competencies

Education: Bachelor's degree in Pharmacy. Experience: N/A. License/Certification: Pharmacist's License, ACLS Certification.
