Description

Under the general supervision of the Director of Pharmacy and the Pharmacy Management Team, the Geriatric Inpatient Pharmacist is responsible for medication regimen review, drug distribution, and providing clinical pharmacy services within the scope of practice.

Core Competencies
  • Assess prescriptions for appropriateness and completeness, documenting changes and clarifications.
  • Serve as a ‘Standards of Behavior’ mentor/role model as outlined in HR Policy 203.
  • Supervise and direct ancillary personnel to ensure timely and accurate pharmacy services.
  • Maintain competencies for compounded sterile and non-sterile products, ensuring proper documentation.
  • Ensure quality assurance for unit-dose, compounded, and sterile products following storage, preparation, labeling, handling, and disposal policies.
  • Provide drug information to healthcare professionals and the public when needed.
  • Maintain medication security according to policies.
  • Handle controlled substances appropriately per policy.
  • Complete annual clinical competencies timely.
  • Provide healthcare services across all age groups, including Neonates, Pediatrics, Adolescents, Adults, and Geriatrics.
  • Respond to emergency code pages and prepare medications as required.
