Job Summary

• The Pharmacist collaborates with the Director of Pharmacy to compound and dispense medications and pharmaceutical supplies based on written prescriptions from physicians.
• Participates in Performance Improvement plans within the hospital.
• Advises Administration, Medical Staff, and hospital staff on quality, regulatory, and risk issues.
• Supports clinical and Medical Staff as a consultant.
• Assists the Director in coordinating pharmacy activities.
• Interprets medication orders, compounds, and dispenses medications in accordance with legal and professional standards.
• Supervises technical support staff.
• Contributes to clinical, quality management, and educational programs.
• Provides support to all departments concerning patient care for adults and the geriatric population (ages 65+).

Essential Functions

• Maintains and disseminates drug information through education, consultation, and collaboration with staff and patients.
• Assists other Pharmacists and Technicians with routine duties.
• Provides guidance to Pharmacy personnel.
• Uses professional judgment to impart therapeutic and drug information.
• Competent in treating adult and geriatric patients.
• Capable of entering at least 40 orders per four-hour shift.
• Generates reports, performs backup procedures accurately.
• Assists Technicians with technical tasks and checks their work prior to medication release.
• Ensures proper storage and documentation of medications and supplies.
• Prioritizes workload effectively, handling STAT, Now, and Waiting orders appropriately.
• Reviews patient profiles to ensure safe and appropriate drug therapy, avoiding therapeutic duplication and allergies.
• Adjusts medication doses based on patient-specific factors like height, weight, renal function, and disease state.
• Maintains knowledge of patient population served and pharmacokinetics dosing.
• Ensures accurate labeling and security of medications and controlled substances.
• Communicates workload and issues to the Director.
• Uses computer systems effectively with basic word processing skills.
• Organized, self-motivated, and capable of managing multiple tasks.
• Reads, writes, and speaks fluent English; maintains good attendance.
• Travel requirements are minimal (0%).

Education

• Graduate of an accredited School of Pharmacy with a Pharmacist degree.

Licenses/Certifications

• BCLS certification.

Experience

• Minimum of three months' experience in pharmacy.
• Knowledge of OSHA, JCAHO, HCFA, and state guidelines.
• Experience facilitating teams and writing policies and procedures.
