Job summary

The Pharmacist role ensures therapy appropriateness, accurate dispensing, documentation, and education to patients and caregivers in line with standards and regulations.

Qualifications

• Pharmacy degree from an accredited college
• Licensed pharmacist in state of employment
• Minimum two years in acute-care or one year in home infusion
• Experience with IV admixture and IV therapy
• Ability to evaluate and monitor drug therapy with healthcare professionals
• Strong problem-solving and supervision skills
• Flexible with work schedule
• Excellent communication skills

Responsibilities

• Demonstrates ethical pharmacy practice and compliance with policies and regulations
• Assess patient suitability for home care and prescribed therapy
• Design patient-specific plans of care and document progress
• Write patient-appropriate orders and ensure billing documentation
• Dispense medications, supplies, and ensure appropriate authorizations
• Submit electronic claims for medications
• Document communication with clinicians and patients in medical records
• Maintain patient medication profiles and pharmacy documentation
• Lead and supervise pharmacy support staff and interns
• Participate in patient care conferences and on-call processes
• Provide training and supervision to staff and interns
• Act as Pharmacist in Charge during absences if needed
• Other related duties as directed
