Job Description: To provide patient-focused pharmacy services including monitoring of therapy, interventions, basic pharmacokinetics, education and medication distribution. To support the mission, activities and programs of the division, department and the Hospital.

  • Participate in clinical care for the broader patient population. Collaborate with the admitting care team to obtain prior to admission medication histories and ensure accurate and safe ordering/dispensing of appropriate home medications or formulary alternatives.
  • Participate in bedside emergency patient care (strokes, traumas and other medical emergencies).
  • Assist with clinical advancement and future growth opportunities for the pharmacy team.
  • Performs other duties and provides patient care in other clinical areas as assigned.
Key Duties & Responsibilities
  • Accurately assess the appropriateness of each medication as prescribed based on the patient’s diagnosis, condition, allergies, and medication history; review for adverse drug reactions and interactions. Promote rational therapy by evaluating orders against current literature, UAB formulary, and knowledge; contact the physician or healthcare professional to recommend changes when appropriate.
  • Monitor patient-specific response to prescribed therapies; re-evaluate prescribed therapy and make recommendations as needed.
  • Document patient medication history and notify appropriate clinicians regarding issues with compliance or therapy.
  • Ensure safe distribution of medications for administration in assigned area; triage orders to meet immediate patient care needs.
  • Provide drug information to patients, families, physicians, nurses and other healthcare professionals. Share clinical observations and new information from literature to optimize medication use.
  • Support educational programs by mentoring pharmacy students and residents.
  • Performs other duties as assigned.

Qualifications: Bachelor’s degree in Pharmacy required. Work experience may not substitute for education. Currently licensed or eligible to be licensed as a Pharmacist by the Alabama Board of Pharmacy (ALBOP) required.
