Job summary

ProHealth Care in Wisconsin seeks a Community Pharmacist to review medication orders, prepare patient profiles, and ensure correct drug, dosage and route. You will verify interactions and allergies, and support safe dispensing, labeling and distribution of all drugs.

Collaborating with clinicians, you ensure patient safety, adhere to standards, and participate in activities aligned with accreditation bodies. Licensure in Wisconsin is required.

Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Active Wisconsin Pharmacist license in good standing.
• ACLS, BLS, and PALS certifications required.
• Valid Wisconsin driver's license.

Responsibilities

• Review and interpret medication orders, ensuring correct drug, dosage and route.
• Prepare patient profiles and check for drug interactions and allergies.
• Safely procure, prepare, label, distribute and control all drugs.
• Notify staff when drug therapy is questionable and support regulatory compliance.
• Participate in pharmacy activities aligned with regulatory agencies.

Job description

What You Will Do:

Community Pharmacist at ProHealth Care accurately reviews and interprets medication orders; prepares accurate and complete patient profiles; reviews profiles for correct drug, dosage and route of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed. Efficiently performs other professional pharmacy duties including the safe procurement, preparation, labeling, distribution and control of all drugs. Notifies appropriate medical/nursing staff whenever appropriate drug therapy is questionable. Actively supports and participates in pharmacy activities recommended by the TJC or other regulatory agencies.

• Doctor of Pharmacy (PharmD)

• Valid WI driver's license and in good standing
• Advanced Cardiac Life Support (ACLS)
• Basic Life Support (BLS)
• Pediatric Advanced Life Support (PALS)
