ProHealth Care is seeking a Community Pharmacist to review medication orders, prepare accurate patient profiles, and verify drug, dosage, and administration routes. The role emphasizes safety, drug interaction checks, allergy screening, and accurate labeling and distribution of medications.

The position requires a PharmD, Wisconsin pharmacist license, and ACLS/BLS/PALS certifications, with a valid Wisconsin driver’s license.

What You Will Do

Community Pharmacist at ProHealth Care accurately reviews and interprets medication orders; prepares accurate and complete patient profiles; reviews profiles for correct drug, dosage and route of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed. Efficiently performs other professional pharmacy duties including the

Community Pharmacist at ProHealth Care accurately reviews and interprets medication orders; prepares accurate and complete patient profiles; reviews profiles for correct drug, dosage and route of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed. Efficiently performs other professional pharmacy duties including the safe procurement, preparation, labeling, distribution and control of all drugs. Notifies appropriate medical/nursing staff whenever appropriate drug therapy is questionable. Actively supports and participates in pharmacy activities recommended by the TJC or other regulatory agencies.

Requirements

• Doctor of Pharmacy (PharmD)
• Pharmacist (R.Ph. or RPh)
• Valid WI driver's license and in good standing
• Advanced Cardiac Life Support (ACLS)
• Basic Life Support (BLS)
• Pediatric Advanced Life Support (PALS)
