Qualifications

Responsibilities

• Lead implementation of strategic pharmacy programs to improve patient health and service.
• Engage patients and providers through outreach, education, and reminders to improve medication adherence.
• Provide clinician education on measure requirements and support Rx therapy optimization.
• Maintain accurate documentation and ensure compliance with regulatory standards.
