Job summary
Lumicera Health Services is seeking a Pharmacist, Clinical Operations to join our Madison team. The role focuses on telephonic consultations regarding specialty medications, medication reconciliation, and ensuring accurate order verification prior to shipment.
The pharmacist will interpret prescriptions, verify details with providers or patients, and help meet production metrics while maintaining a patient-centered care model.
Qualifications
- Graduate of ACPE Accreditation School of Pharmacy with a B.S. Pharmacy or PharmD degree.
- Current, unrestricted Pharmacist license in state where on-site work will be conducted; must maintain education credits.
- At least 2 years of experience as a practicing licensed Pharmacist preferred.
- Specialty pharmacy experience or understanding REMS and Limited Distribution Products preferred.
- Participate in, adhere to, and support compliance program objectives.
- Ability to interact cooperatively and respectfully with other employees.
Responsibilities
- Verify prescriptions for pharmaceutical preparations and devices; maintain records; consult with physicians; advise on formulary medications and cost-effective therapies.
- Provide clear clinical documentation of patient consultations in pharmacy software; ensure URAC and EMR compliance.
- Provide oversight and quality assurance to pharmacy technicians.
- Interpret and evaluate prescriptions for specialty medications; contact providers/patients to verify information.
- Work with providers to convert prescriptions to generics or preferred drugs when possible.
- Verify information entered in the system; answer clinical questions from staff.
- Stay current with advancements in specialty therapies and new drug approvals.
- Provide patient education via telephonic consultations per Lumicera Patient Management Program.

Lumicera Health Services is seeking a Pharmacist, Clinical Operations to join our team!
The Pharmacist, Clinical Operations will be responsible for performing telephonic consultations with patients regarding the use of their specialty medication and management of their disease state. They will perform medication reconciliation, screen for potential drug interactions, and provide side effect management recommendations. Another main duty of this role will include a high volume of product verification to ensure accuracy of orders prior to shipment. The Pharmacist, Clinical Operations can expect to interpret prescribers’ prescriptions, contact prescribers and/or patients to verify information on prescriptions, and expedite the processing of orders. The individual in this position will be expected to meet department production metrics and service levels, support quality standards, resolve patient issues, and vary work priorities and activities to accommodate business needs. The Pharmacist, Clinical Operations works independently with minimal guidance and represents the organization in using patient-centered care to enhance the patient experience.

- Verify prescriptions for pharmaceutical preparations and devices for patients at company facilities; maintain accurate and detailed records of all prescription transactions, and submit appropriate claims for third-party reimbursement; consult with physicians and nurses regarding pharmaceutical questions; advise physicians on formulary medications and cost-effective and innovative therapies; answer pharmaceutical questions from patients and staff; monitor storage, distribution, and use of pharmaceuticals.
- Provide clear, detailed clinical documentation of patient consultations and interventions in pharmacy software in compliance with internal policies and procedures and external regulatory bodies (such as URAC) and/or EMR systems efficiently.
- Provide oversight and quality assurance to pharmacy technicians.
- Interpret and clinically evaluate prescriptions for specialty medications based on available information and resources. Contact providers and/or patients to verify information related to prescriptions such as drug strength, intended directions, and diagnosis to expedite processing of orders. Verify and confirm validity of controlled substances.
- Contact providers for new prescriptions, consult with prescribers and nurses regarding clinical questions, and contact pharmacies for prescription transfers. Work with providers to convert prescriptions to generic, biosimilar, or preferred drugs whenever possible.
- Verify prescription information entered in the system by data entry or order entry. Answer clinical questions from the pharmacy team.
- Adapt to new workflows and processes. Solve unique pharmaceutical problems. Stay current with advancements in specialty pharmacy therapies, including new drug approvals and specialty disease state treatment guidelines.
- Provide patient education through telephonic consultations for patients of the specialty pharmacy in accordance with Lumicera’s Patient Management Program. The consultations will focus on effective assessment and communication of disease state management, side effect mitigation strategies, administration of medication, appropriate storage, and therapeutic appropriateness, while following a patient-centered care model. Answer clinical questions from patients and caregivers.
- Document and report medication errors in accordance with company policies and manufacturer contracts. Facilitate error reduction and prevention methods based off errors and ISMP standards. Adhere to all Lumicera pharmacovigilance standards.
- Contribute to the quality management program through daily operations and efforts toward sustaining key pharmacy performance metrics. Participation in quality improvement activities through routinely sharing ideas for improvements or implementing improvement opportunities with supervisors or quality team members
