Baptist Health in Jacksonville, FL, is seeking a Pharmacist to join our Oncology team at Baptist MD Anderson. This role focuses on patient‑centered medication management within an interdisciplinary team, in both ambulatory and inpatient settings as clinically appropriate.

The successful candidate will provide clinical pharmacy support, contribute to formulary management and pathway development, and participate in education for patients, families, and healthcare professionals.

Formación

• Florida pharmacist license required.
• In-patient hospital pharmacy experience preferred.
• PGY1 residency preferred.
• Bachelor of Science degree required.

Responsabilidades

• Provides safe, evidence-based, and patient-centered medication management as part of an interdisciplinary team.
• Offers clinical pharmacy support to physicians, nurses, and staff in ambulatory clinics and inpatient settings when clinically appropriate.
• Recommends, designs, implements, monitors, and modifies pharmaceutical care plans to optimize outcomes.
• Utilizes extensive clinical experience in disease-state management, formulary, pathway/guideline development, policy development, and oncology reimbursement.
• Educates patients, family members, healthcare providers, and pharmacy students/residents.

Baptist Health is currently looking for a Pharmacist to join our Oncology team at Baptist MD Anderson!

As a Pharmacist, You Will Be Responsible For

• Provides safe, evidence-based, cost effective, and patient centered medication management as an integral part of an interdisciplinary team.
• Responsible for providing clinical pharmacy support to physicians, nurses, pharmacists, and other staff when caring for patients in the ambulatory clinics, and while inpatient, if clinically appropriate.
• A high level of expertise is utilized to recommend, design, implement, monitor, and modify pharmaceutical care plans to optimize patient outcomes.
• Extensive clinical experience within their disease state specialization as well as pharmacy, practice experience including medication safety, formulary management, pathway/guideline development, policy development/implementation, oncology reimbursement and coverage, and clinical research.
• Have an active role in the education of patients and family members, healthcare providers, and pharmacy students and residents.

Education & Credential Requirements

• Registered Pharmacist - State Board of Pharmacy Required
• Licensed Pharmacist - DOH Required
• Bachelor of Science Required

A Pharmacist license within the State of Florida is required. In-Patient Hospital Pharmacy Experience is preferred and PGY1 Residency is preferred.

This Pharmacist position will be located at Baptist MD Anderson.
