Position Title

Clinical Cardiology Pharmacist

Position Purpose

The Clinical Cardiology Pharmacist provides comprehensive pharmaceutical care services to patients within cardiology and related specialty areas. This role supports multidisciplinary healthcare teams by optimizing medication therapy, improving clinical outcomes, and ensuring compliance with federal, military, and accreditation standards.

Duties and Responsibilities
  • Provide clinical pharmacy services for patients with cardiovascular conditions, including heart failure, acute coronary syndrome, arrhythmias, and hypertension
  • Review, monitor, and optimize medication therapy based on labs, diagnostics, and evidence-based guidelines
  • Participate in multidisciplinary rounds and collaborate with physicians, nurses, and care teams to develop treatment plans
  • Manage and monitor anticoagulation therapy (e.g., warfarin, DOACs, heparin)
  • Identify and prevent adverse drug events and ensure safe medication practices
  • Educate patients and caregivers on medication use, adherence, and disease state management
  • Provide drug information and clinical guidance to healthcare staff
  • Support formulary management, protocol development, and quality improvement initiatives
  • Ensure compliance with The Joint Commission and Department of Defense standards
  • Demonstrate professional communication with patients, family members, professional and support personnel involved in the delivery of patient care.
Requirements
  • Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND completion of ASHP-accredited PGY2 in Cardiology OR
  • Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND 3 years of practical clinical practice experience in Cardiology with BPS board certification.
  • American Heart Association Basic Life Support (BLS) certification.
Required Knowledge and Experience
  • Minimum of one year of pharmacy experience within the last three years
  • High-level of reliability and dependability
  • A valid driver's license is required
Physical Requirements

The position requires repetitive motion, such as standing, walking, sitting and bending. Light lifting, up to 20 pounds, may be required.
