Overview
In collaboration with patients, families (as defined by the patient), and staff across all disciplines and departments, the pharmacist is responsible for providing pharmaceutical care services.
Responsibilities
PRIMARY ACCOUNTABILITIES
- Ensure prescriptions are dispensed safely and accurately as prescribed.
- Conduct patient drug regimen reviews.
- Provide patient prescription counseling services as required.
- Prepare and maintain required records per regulations.
- Communicate effectively with patients, providers, and other clinical staff.
- Perform other duties as assigned.
- Ensure all tasks related to patient care, administrative processes, and related duties comply with regulatory and accreditation standards, including The Joint Commission and CommUnityCare policies.
- Develop and maintain positive relationships and partnerships with co-workers.
- Interact respectfully and collaboratively with patients and their families, aiming to develop favorable relationships.
- Collaborate with the care team to provide patient-centered care.
- Maintain professionalism in all actions, job performance, conduct, and communications, representing CommUnityCare appropriately.
- Ensure compliance with all company policies, procedures, mission, and values.
- Complete mandatory training and maintain appropriate credentials and licensure.
- Build strong relationships and deliver excellent customer service.
- Possess a high level of knowledge in drug therapy and medication dispensing.
- Have basic computer skills, including Microsoft Office.
- Pay close attention to detail and accuracy.
- Exhibit excellent verbal and written communication skills.
Qualifications
Education:
- Bachelor’s degree in Pharmacy (BS Pharmacy) or Doctor of Pharmacy (PharmD) required.
- PharmD preferred.
Experience:
- At least two (2) years of community pharmacy experience required.
- Experience as a technician or intern may count towards experience.
- Five (5) years of pharmacy experience in a community clinic setting preferred, especially with Medication Therapy Management.
Licensure and Certifications:
- Registered Pharmacist in good standing with the Texas State Board of Pharmacy.
- Current CPR certification through American Heart Association or Red Cross, maintained during employment.
