Qualifications

• Graduate of an ACPE-accredited College of Pharmacy.
• B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree.
• Current CPR/BLS Certification from the American Heart Association or American Red Cross.
• Board Certification is highly desirable.
• Completion of a Pharmacy Residency.
• A minimum of 2 years in ambulatory pharmacy or equivalent years of experience.

Responsibilities

• Collaborating with providers, pharmacists, and nursing teams to ensure safe and effective patient care.
• Assists pharmacy leadership with necessary records as it pertains to Alabama Board of Pharmacy requirements for compliance.
• Serves as a drug information resource for providers regarding appropriate medication use in the outpatient setting; provide educational presentations or in-services.
• Assists in development, administration, and improvement of ambulatory pharmacy programs and services.
• Assists with drug inventory management and Omnicell systems maintenance and coordination.
• Works with dispensing pharmacists on access issues and keeps knowledge up-to-date with drug developments and guidelines.
• Develops workflow patterns for timely outpatient ambulatory services.
• Executes therapeutic plans with cost-effective treatments and patient assistance programs.

Description

GENERAL SUMMARY

Responsible for implementing and providing clinical pharmacy services at Cooper Green Mercy Health Services Authority (CGMSHA). The pharmacist works collaboratively with prescribers to facilitate optimal patient outcomes regarding safe medication use.

KEY RESPONSIBILITIES

1. Interpret and evaluate physician order review and verification for all medications.
2. Collaborating with providers, pharmacists, and nursing teams to ensure safe and effective patient care.
3. Assists pharmacy leadership with necessary records as it pertains to Alabama Board of Pharmacy requirements for compliance.
4. Serves as a drug information resource for providers regarding appropriate medication use in the outpatient setting. Share pharmaceutical expertise by coordinating and providing educational presentations or in-services for pharmacy and non-pharmacy staff.
5. Assists in the development, administration, and improvement of ambulatory pharmacy programs and services.
6. Assists with drug inventory management by monitoring stock levels, conducting routine inventory counts, and ensuring timely replenishment of medication.
7. Provides support for Omnicell automated dispensing systems, including troubleshooting, inventory management, restocking, issue assistance, and coordination of maintenance activities as directed by pharmacy leadership.
8. Works laterally with dispensing pharmacists when issues are identified related to medication access.
9. Maintains up-to-date knowledge of drug development and standard of care guidelines, biosimilar launches, safety alerts, and treatment protocols.
10. Develops and maintains workflow patterns that assure timely delivery of outpatient ambulatory pharmacy services.
11. Executes therapeutic plans utilizing the safest and most cost-effective medication treatments, including extensive utilization of patient assistance foundations and support programs.
12. Performs quality control on medication orders and prepared medications, and additional quality control activities as required by pharmacy leadership.
13. Provides oversight of population-level prescribing and pharmaceutical care trends for interventions required by quality measures, cost containment initiatives, or necessary for improved patient care as identified by pharmacy leadership.
14. Serves on departmental and organizational committees, contributing expertise and recommendations to enhance pharmacy services and patient care.
15. Performs other duties as assigned.
