Sanford Health is seeking an Adult Internal Medicine Focus Pharmacist to provide direct pharmaceutical care to adult inpatients on medical and surgical units. You will work in a decentralized model with providers and nurses to optimize therapy and ensure safety.

The role includes prospective reviews, rounds participation, dosing guidance, education for staff and patients, and active involvement in medication safety and regulatory adherence.

Qualifications

• Graduate of an accredited School of Pharmacy; knowledge of drugs and state regulations.
• ASHP-accredited PGY1 residency or equivalent experience preferred.
• License in good standing by State Board of Pharmacy; multi-state licensure as required.

Job Description

The Adult Internal Medicine Focus Pharmacist provides direct pharmaceutical E9care to adult inpatient populations on medical and surgical units. Decentralized to patient care areas, this pharmacist works closely with providers, nurses, and other interdisciplinary team members to optimize medication therapy and promote safe, effective, evidence-based care.

Key responsibilities include prospective patient profile review, participation in interdisciplinary rounds, identification and resolution of medication-related problems, patient education, and clinical consultation for healthcare professionals. The pharmacist also performs medication order verification, evaluates medication therapy for accuracy and appropriateness, and provides guidance regarding dosing, drug interactions, therapeutic monitoring, adverse effects, and medication administration.

This role supports operational and distributive pharmacy functions as needed while promoting medication safety, regulatory compliance, and continuity of care in a fast-paced healthcare environment.

Qualifications

In a medical center or institutional setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.

Graduate of an accredited School of Pharmacy. Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience preferred.

License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
