Job summary

PharMerica in Greensburg, PA is hiring a full-time Pharmacist to join our closed-door, non-retail pharmacy serving long-term care and senior living clients. You’ll evaluate orders, provide clinical recommendations, maintain patient profiles, and collaborate with physicians and nurses while ensuring compliance with state and federal regulations.

This role emphasizes independent work within a supportive team, ongoing training, and opportunities for career growth.

Qualifications

• Bachelor’s Degree in Pharmacy, PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, safety practices, regulatory standards.
• Proficiency in EMR and pharmacy information systems.
• Strong clinical judgement and communication skills.
• Position will be posted for at least 7 business days.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff for therapy changes.
• Maintain up-to-date patient profiles including medical history and current medications.
• Ensure monitoring lab tests for high-risk meds are ordered and communicated.
• Collaborate with physicians and nurses for clinical and technical support.
• Document communications and interventions in the medical record.
• Implement and enforce policies for medication handling and safety.
• Ensure compliance with state and federal regulatory requirements.
• Report medication errors and adverse events to Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.

As a Pharmacist you will

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
• Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
• Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
• Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
• Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
• Perform other pharmacy-related tasks and responsibilities as assigned.

Qualifications

Required:

• Bachelor’s Degree in Pharmacy, Pharm D preferred
• One year experience in long term care desired
• License/Certification(s): current and valid pharmacist license in the state of practice
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems
• Strong clinical judgement, and effective written and verbal communication
• Position will be posted for a minimum of 7 business day
