Job summary

Remedi SeniorCare, a division of Clarest Health, is seeking a Staff Pharmacist for our closed-door LTC pharmacy in Troy, OH. You will ensure clinical accuracy and timely medication delivery, collaborating with Pharmacy Technicians and physicians to optimize patient care.

This full-time role offers a flexible 4–10 schedule (3:00 PM–1:30 AM), competitive starting salary range of $120,000–$131,000, comprehensive health benefits, 401k, PTO, and opportunities for growth within a mission-driven

Qualifications

• PharmD or equivalent degree with an active state license.
• Experience with intravenous admixtures and intravenous therapy is preferred.
• Minimum 1 year of licensed pharmacist experience in a healthcare setting.
• Long-Term Care pharmacy experience preferred.
• Familiarity with Frameworks and DocuTrack systems preferred.

Responsibilities

• Process new patient referrals, dispense and monitor medications.
• Work with Pharmacy Technicians in preparation and dispensing of medications.
• Provide client care including patient follow-up and dosing discussions.
• Ensure compliance with local, state, and federal reporting requirements.
• Maintain safety practices and enforce company policies.
• Adhere to Remedi’s Code of Conduct and report violations.

Salary : Starting Range of $120,000 - $131,000 (Dependent on Experience Level)

Key Responsibilities

• Processing new patient referral prescriptions, preparation, dispensing, and monitoring of medications
• The Pharmacist will work closely with Pharmacy Technicians in the preparation and dispensing of medications
• The Pharmacist will work closely with the Pharmacy Technicians to provide client care to include, but not limited to, patient follow-up, monitoring clinical laboratory data, working with physicians on dosing antibiotics, etc.
• Ability to perform accurately and efficiently when imputing information.
• Ensure compliance with local, state, and federal reporting requirements.
• Assists with ensuring safety practices and procedures are adhered to and practiced.
• Enforces all company and departmental policies and procedures.
• Comply with all applicable state and federal regulations related to pharmacy practices.
• Customer Focus - Is dedicated to meeting the expectations and requirements of internal and external customers; gets first‑hand customer information and uses it for improvements in products and services; talks and acts with customers in mind; establishes and maintains effective relationships with customers and gains their trust and respect.
• The ability to adhere to Remedi’s Code of Conduct, follow Remedi Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer

Qualifications/Requirements

• PharmD or equivalent degree in Pharmacy; active state license in good standing
• Experience and knowledge base of intravenous admixtures services and intravenous therapy desired.
• Minimum 1 year of experience as a licensed pharmacist in a healthcare setting
• Long-Term Care (LTC) pharmacy experience preferred
• Familiarity with Frameworks and DocuTrack systems strongly preferred
• Ability to lift up to 40 lbs and stand for extended periods during shifts
