Job summary

Bayhealth is seeking a pharmacist at Sussex Campus Hospital to oversee safe medication distribution and provide Specialty Pharmacy services in a fast-paced hospital setting. The role focuses on ensuring accuracy, safety, and compliance with policies and accreditation requirements.

Responsibilities

  • Review and process medication orders for safety and accuracy.
  • Provide drug information to medical staff and respond to inquiries.
  • Participate in multidisciplinary rounds and clinical programs.
  • Supervise technicians and ensure proper dispense preparation and compliance.
  • Support quality improvement and safety initiatives within the department.
  • Account for controlled substances and report adverse events.
  • Perform other duties within scope as assigned.

General Summary

The pharmacist is a healthcare professional responsible for safe, effective, timely medication distribution and administration.

Provides Specialty Pharmacy services, consistent with policies and accreditation requirements.

Responsibilities
  • Medication/Solution Order Review and Processing - Reviews prescriber's orders for appropriateness relative to the patient's condition, height, weight, allergies and age. Review prescriber's orders for appropriateness and compliance with legal requirements, institutional policies and procedures, and drug use restrictions in accordance with prescribing limits set by the Pharmacy and Therapeutics Committee and the Medical Executive Committee. Reviews medication orders for duplicate therapy, drug-drug interactions, dosage range, IV incompatibility and food-drug interaction to facilitate accurate and timely medication/solution distribution and administration.
  • Drug Information Resource - Provides pertinent drug information to medical and ancillary hospital staff. Researches medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Responds timely and in a professional manner.
  • Departmental Clinical Program Participation - Completes pharmacy consults in a timely and accurate manner. Attends multi-disciplinary rounds.
  • Technician and Ancillary Staff Supervision - Reviews medications prepared for distribution by technicians with respect to accuracy, appropriateness, and timeliness. Monitors technician accountability to follow established policies and procedures for medication delivery, including the automated dispensing system. Reviews technician preparation of intravenous admixtures, parenteral nutrition compounding, chemotherapy and miscellaneous sterile products. Ensures and validates processes are followed for dispense prep of medications. At a minimum, departmental dispense prep must be above 84 % and one technician pearl must be identified and completed to achieve successful.
  • Departmental Performance Improvement Participation - Participates in the departmental improvement process on a routine basis by ensuring that all quality control measures are followed and properly documented. This includes but is not limited to: monitors ADC Report Reviews (ex: override and discrepancies), monthly narcotic inspections. Supports the High Reliability Organization activities (ex: Safety First Reports, Near Misses, Performance Improvement)
  • . Controlled Substance(s) Accountability - Completes all chain of custody related processes in regards to the dispensing of controlled substances. This includes but is not limited to: appropriate dispensing, storage and hand-off of medications prior to delivery, and monitoring, investigating and clearing of all open transactions involving controlled substances.
  • Adverse Drug Reporting - Reviews, identifies, reports and documents potential ADRs.
  • All other duties as assigned within the scope and range of job responsibilities
