Qualifications

• Must have Pharmacist license in Indiana.
• Must have recent inpatient experience.
• Experienced Pharmacist.

Responsibilities

• Dispense prescription medications to patients and ensure proper dosage and administration.
• Review and interpret physician orders to ensure accuracy and appropriateness.
• Monitor patient responses to medications and adjust treatment plans as necessary.
• Provide drug information and education to medical staff and patients.
• Conduct medication reconciliation upon patient admission and discharge.
• Manage inventory of pharmaceuticals and ensure proper storage conditions.
• Collaborate with healthcare team to develop and implement individualized patient care plans.
• Perform clinical rounds with physicians and other healthcare providers.
• Participate in clinical research and trials as needed.
• Supervise pharmacy technicians and other support staff.
• Ensure compliance with regulatory requirements and hospital policies.
• Maintain patient records and document medication therapy outcomes.
