Baptist Health Lexington Pharmacy Services is seeking a Pharmacist for First and Second Shifts to support inpatient and ambulatory services in Lexington, KY. The role focuses on accuracy in medication orders, anticoagulation management, and collaborative care with clinical teams.

Qualified candidates will be graduates of an accredited College of Pharmacy with a Kentucky license or eligibility and BLS certification; 1–3 years experience or ASHP residency preferred.

Qualifications

• Graduate of accredited College of Pharmacy.
• Kentucky Board of Pharmacy license in good standing.
• 1-3 years related work experience or ASHP-accredited residency preferred.
• Basic Life Support (BLS) certification required; obtain within 30 days of hire.

Responsibilities

• Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served.
• Ensures drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcomes.
• Provides comprehensive decentralized or pharmacy clinical services including anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
• Works with inter-professional patient care teams to help patients with transitions to care, including discharge preparation.
