Job summary

PharMerica is seeking a full-time Pharmacist in Fort Lauderdale, FL, to support long-term care and senior living clients in a non-retail environment. Responsibilities include analyzing medication orders, providing clinical recommendations, and maintaining comprehensive patient records.

Qualifications

  • Bachelor’s degree in Pharmacy; PharmD preferred.
  • One year of experience in long-term care is desired.
  • Current pharmacist license in state of practice.

Responsibilities

  • Evaluate physician medication orders for drug appropriateness, dosage accuracy, interactions, and route of administration.
  • Provide clinical recommendations to physicians and healthcare staff on medication therapy changes.
  • Maintain up-to-date patient/resident profiles with medical history, labs, and current medications.
  • Ensure lab tests for monitoring high-risk meds are ordered, reviewed, and communicated.
  • Collaborate with physicians, nurses, and healthcare professionals for clinical and technical support.
  • Document communications and interventions in the patient's medical record, including family interactions.
  • Enforce policies for safe handling, dispensing, and management of medications.
  • Ensure compliance with state and federal pharmacy regulations.
  • Report medication errors to Pharmacy Manager as required.
  • Educate and support clinical staff on safe medication handling procedures.
  • Perform other pharmacy-related tasks as assigned.

PharMerica is hiring a full-time Pharmacist in Fort Lauderdale, FL. The position is in a non-retail environment dedicated to meeting the pharmaceutical needs of long-term care and senior living clients.

Schedule: 4, 10 hour shifts

M-F: 6am-4:30pm and 12:30pm-11:00pm

Sa-Su: 6am-4:30pm rotating weekends * every 3rd weekend

Responsibilities
  • Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
  • Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
  • Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy-related tasks and responsibilities as assigned.
Qualifications
  • Bachelor’s Degree in Pharmacy, Pharm D preferred
  • One year experience in long term care desired
  • License/Certification(s): current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement, and effective written and verbal communication
  • Position will be posted for a minimum of 7 business day
