Qualifications

• Pharm.D. degree required.
• 1+ years of experience preferred; new graduates are encouraged to apply.
• Active Louisiana Pharmacist License or eligibility.
• Ability to pass background check, health screen, and drug test.

Responsibilities

• Review and verify medication orders for long-term care residents for accuracy.
• Conduct comprehensive drug regimen reviews and identify issues.
• Collaborate with physicians, nurses, and healthcare teams to optimize therapy.
• Monitor for drug interactions, contraindications and adverse effects.
• Ensure compliance with federal and state long-term care regulations.
• Provide consultant pharmacist services and recommendations to staff.
• Attend interdisciplinary care planning meetings and share clinical expertise.
• Maintain documentation compliant with regulatory requirements.
• Educate staff on proper medication administration and storage procedures.
• Respond to medication-related inquiries with drug information.
• Oversee storage, handling, and disposal of medications.

Job Specifics and Requirements:
