Job summary

Altev Community Pharmacy is seeking a Part-time Pharmacist to join our team in Kentucky. This role enables practice at the top of your license and leverages Ohio provider status to expand access to care.

The ideal candidate is clinically driven, adaptable, and committed to advancing community pharmacy practice through patient-centered services and collaboration with local providers.

Qualifications

• Active and unrestricted Ohio Pharmacist license.
• Strong knowledge of clinical pharmacy practices.
• Excellent communication and interpersonal skills.
• Prior experience in independent/community pharmacy preferred.
• Flexible availability to work PRN hours as needed.

Responsibilities

• Dispense prescriptions accurately and in compliance with regulations.
• Deliver MTM, immunizations, point-of-care testing, and disease state management.
• Provide billable clinical services via Ohio provider status.
• Support adherence packaging and Long Term Care at Home programs.
• Counsel patients and educate on medication use, adherence, and lifestyle interventions.
• Collaborate with prescribers, nurses, and community partners to close gaps in care.

Key Responsibilities/ General Accountabilities:

• Accurately and efficiently dispense prescriptions in compliance with all state and federal regulations.
• Deliver patient-centered clinical services including MTM, immunizations, point-of-care testing, and disease state management.
• Leverage Ohio pharmacist provider status to provide billable clinical services.
• Support innovative programs such as adherence packaging and Long term Care at Home to improve patient outcomes.
• Provide patient counseling and education on medication use, adherence, and lifestyle interventions.
• Collaborate with prescribers, nurses, and community partners to close gaps in care.
