Job summary

East Houston Medical Center in Houston, TX is seeking an experienced Pharmacist to join our clinical team. The Pharmacist ensures safe, evidence-based pharmaceutical care in accordance with regulations and hospital policies, supporting clinical teams and optimizing patient outcomes.

Qualifications

  • Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy required.
  • Minimum of 2 years of hospital or clinical pharmacy experience preferred.
  • Excellent communication, problem-solving, and interpersonal skills.
  • Commitment to ethical standards and evidence-based practice.

Responsibilities

  • Review, verify, and dispense medications in accordance with prescribed orders and hospital protocols.
  • Provide clinical oversight for medication use, including dosing adjustments, therapeutic drug monitoring, and adverse event prevention.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal pharmacotherapy and patient safety.
  • Counsel patients and families on proper medication use, side effects, and adherence strategies.
  • Maintain accurate pharmacy records and comply with all Board of Pharmacy and regulatory requirements.
  • Participate in quality improvement initiatives, formulary reviews, and staff education programs.
  • Stay current on clinical developments, emerging drug therapies, and best practices in pharmacy care.
