Job summary

Griffin Health Brand is seeking a Clinical Pharmacist to conduct comprehensive medication reviews and support chronic disease management within our clinic.

You will assess regimens, identify drug-related problems, develop individualized action plans, and educate patients and caregivers on medication purpose, benefits, risks, and monitoring requirements. Collaboration with healthcare teams is essential.

Qualifications

• Perform comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs).
• Assess medications for appropriate selection, dose, route, frequency, indication, therapeutic effectiveness, and safety.
• Develop medication action plans and communicate recommendations to providers.

Responsibilities

• Provide pharmacist-led interventions and monitoring for chronic diseases.
• Educate patients and caregivers on medication purpose, benefits, risks, monitoring, and lifestyle changes.
• Support patients in understanding and managing chronic conditions and improving adherence.

Clinical Pharmacist Assessment

Evaluate patient-specific factors including:

• Drug-disease interactions
• Allergies and intolerances
• Renal function considerations

• Contraindications and precautions
• Duplicate therapy
• Medication affordability and access barriers

Patient Care & Education

• Provide medication counseling and education to patients and caregivers
• Promote medication adherence and self-management strategies
• Educate patients regarding:
  • Medication purpose
  • Expected benefits
  • Potential adverse effects
  • Monitoring requirements
  • Lifestyle modifications
• Support patients in understanding and managing chronic conditions

Chronic Disease Management Support

Provide pharmacist-led interventions and monitoring for conditions such as:

• Diabetes
• Anticoagulation therapy
• Asthma/COPD
• Other chronic disease states as identified by clinic needs
