Job summary

InfuCare Rx in Cumberland, MD is seeking a Pharmacist to support home infusion therapies. You will interact with insurance companies, physicians, nurses, and patients to ensure safe and effective drug therapy and to resolve issues as they arise.

You will participate in distributive and clinical pharmacy activities, monitor outcomes, and document all professional activities while maintaining compliance with applicable laws and standards.

Qualifications

  • Required PharmD or BS in Pharmacy from an accredited college.
  • Licensed as a pharmacist in the state of employment.
  • Minimum two years acute-care or one year home infusion therapy experience.
  • Experience with intravenous admixture services and IV therapy, if applicable.
  • Ability to evaluate and monitor drug therapy with the health care team.
  • Excellent communication with patients, families, and professionals.

Responsibilities

  • Adhere to policies and procedures and coordinate referrals/admissions with nursing, customer service, and distribution.
  • Review and verify new patient orders for accuracy and consistency.
  • Collaborate with physicians and nurses to plan safe drug therapy in home infusion.
  • Assess patient, drug, and disease information to identify issues and needs.
  • Monitor therapy outcomes using lab and clinical data and report results to the care team.
  • Maintain thorough documentation in patient records and prepare progress reports for physicians.
  • Oversee sterile compounding and ensure compliance with laws and standards.

Job Description

As a Pharmacist, you’ll be involved with multiple facets of the home infusion process as you interact with insurance companies, staff members, distributors, Physicians/Nurses, and patients to ensure accurate and safe quality service. You will use your experience as part of the Pharmacy Team to resolve problems when unforeseen roadblocks arise. Perform specific distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies.

Primary Duties
  • Adheres to organization policies and procedures.
  • Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution.
  • Receives, reviews, and verifies all new patient orders and documents are the same on the order form.
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy.
  • Conducts initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
  • Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information.
  • Communicates the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate persons.
  • Programs infusion control devices as indicated.
  • Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drug and their use.
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Participates in the preparation of progress reports for the referring physician on a regular, periodic basis.
  • Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state, and local laws and standards of practice which govern the profession of pharmacy.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Will serve as a designated person for all sterile compounding activities, if applicable.
  • Maintains pharmacy records in accordance with applicable laws and regulations.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and conferences.
  • Participates in on-call rotation if applicable.
  • Performs marketing support functions as requested.
  • Participates in orientation/training programs for new personnel and current staff.
  • Participates in departmental QA activities.
  • Attends designated department meetings as scheduled.
  • Performs other tasks or special projects as requested by management.
Qualifications
  • A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy.
  • Currently Licensed as a registered pharmacist in the state of employment.
  • Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy.
  • Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy, if applicable.
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
  • Can work effectively with others to provide safe and effective drug therapy.
  • Can communicate, both verbally and written, with patients, clients, management, and other employees.
  • Can problem solve and supervise technical and other support staff.
  • Must be flexible with work schedule.
  • Possess good organizational and time-management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Dependable in attendance and job performance.
Physical Demands

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
