Job summary

Qualifications

Responsibilities

  • Greet and communicate with patients in a trauma-informed, respectful, and culturally responsive manner.
  • Provide ready-to-pick-up medications to patients, including verifying order readiness in the pharmacy system.
  • Assist in scheduling follow-up appointments as directed by integrated health team members.
  • Package and process medication orders for site or home delivery.
  • Obtain and organize documentation for pharmacists, including insurance paperwork and patient assistance programs.
  • Record clinical services and interactions in the EHR and RXQ systems.
  • Maintain current documentation in EHR and RXQ.
  • Support general admin duties including scanning and document processing.
  • Maintain up-to-date client insurance status and required authorizations.
  • Use EHR, RXQ, and system upgrades as directed by leadership.
  • Deliver excellent customer service to patients, providers, and staff.
  • Maintain confidentiality per HIPAA and protocols.
  • Perform other duties as assigned.

Position Summary

This position supports pharmacy operations by performing a variety of clerical, administrative, and customer service functions in accordance with applicable state and federal regulations. Responsibilities include assisting patients with medication pick‑up, scheduling follow‑up appointments, supporting medication delivery processes, and maintaining accurate documentation for third‑party reimbursement and patient assistance programs. This role ensures patients have access to their medications and care coordination while supporting efficient pharmacy operations and providing front‑line administrative and customer service support.

Duties/Responsibilities (Essential Functions)
  • Greet and communicate with patients in a trauma‑informed, respectful, and culturally responsive manner.
  • Provide ready‑to‑pick‑up medications to patients, including verifying order readiness through the pharmacy operating system.
  • Assist in scheduling follow‑up appointments as directed by integrated health team members.
  • Package and process medication orders for site or home delivery.
  • Obtain and organize documentation for pharmacists, including paperwork for insurance, billing, and patient assistance programs.
  • Record clinical services, communications, and other interactions in the Electronic Health Record (EHR) and pharmacy operating system (RXQ) in a timely and complete manner.
  • Maintain current and accurate documentation in both EHR and RXQ platforms.
  • Support general administrative duties including managing tasks, scanning, PAQ, and other document processing to maintain a complete and current patient chart.
  • Maintain up‑to‑date client insurance status in the system and complete CCARs/DACODs and other authorization documentation as required.
  • Utilize EHR, RXQ, and any future system upgrades as directed by pharmacy leadership.
  • Deliver excellent customer service to patients, providers, internal staff, and external partners.
  • Maintain a respectful, inclusive, and collaborative work environment aligned with CCH's mission and values.
  • Maintain strict confidentiality of patient information in accordance with HIPAA and organizational protocols.
  • Performs other duties as assigned.
Education, Licensure and Certifications
  • High School diploma, GED or equivalent
Experience
  • At least 6 months of experience in a customer service or administrative support role required.
  • Experience working in a healthcare, pharmacy, or community health setting preferred.
Job Competencies
  • Sound critical thinking and decision‑making skills.
  • Strong customer service orientation with excellent interpersonal and communication skills; ability to maintain a high degree of emotional intelligence.
  • Demonstrated sensitivity to and desire to work with underserved populations, including people experiencing homelessness, mental illness, or substance use disorders.
  • Ability to navigate and use Electronic Health Records (EHR) and pharmacy systems (e.g., RXQ).
  • Familiarity with insurance documentation, patient authorizations, and clinical records maintenance preferred.
  • Ability to prioritize multiple responsibilities, meet deadlines, and work independently while collaborating effectively as part of a team.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook), office technology, and general computer use.
  • Bilingual (English/Spanish) preferred.
