Job summary

Independence First in Wisconsin is seeking a dedicated Personal Care Worker to assist consumers with daily living activities, including bathing, feeding, mobility, and grooming, under supervision. The role requires maintaining privacy and accurate documentation in line with care plans.

This position emphasizes confidentiality, following written plans of duties, and providing compassionate one-on-one support to consumers in their homes or care settings.

Qualifications

• Training in personal care services is required or prior experience may be accepted in lieu of formal training.

Responsibilities

• Assist consumer with getting in and out of bed.
• Assist with toileting/incontinence care.

• Care for eyeglasses and hearing aids.

• Simple transfers bed to chair or wheelchair and reverse.
• Skin care (excluding wound care).
• Meal preparation and serving.
• Light cleaning in areas used during personal care activities.
• Change bed linens and wash consumer's personal clothing.

Position Summary

Personal Care Worker provides personal care services to consumer(s) of IndependenceFirst. Personal Care Worker is directly supervised by the Care Coordinator and the consumer(s) for whom they provide personal care services.

Essential Functions of the Job

1. Assisting consumer with getting in and out of bed.
2. Assisting consumer with toileting/incontinence care
3. Assisting consumer with bathing or showering
4. Assistance with eating.
5. Assistance with grooming to include mouth, denture, hair, shaving and nail care.
6. Assistance with dressing and undressing.
7. Care of eyeglasses and hearing aids.
8. Assistance with mobility and ambulation including use of gait belt, walker or cane.
9. Simple transfers including bed to chair or wheel chair and reverse.
10. Skin care excluding wound care.
11. Meal preparation and serving.
12. Light cleaning in areas used during personal care activities.
13. Changing of consumer's bed, washing bed linens and consumer's personal clothing.

Additional Responsibilities

1. Completion in a thorough, accurate and timely manner of all time sheets and documentation of consumer's condition as necessary.
2. Keep consumer's information confidential.

Experience and Skills

Minimum Qualifications

Personal Care Worker must have adequate training in the provision of personal care services and in each skill that the personal care worker is assigned. Documentation of training is required. Previous experience may be accepted in lieu of formal training.
