Job Description:

Position Summary: Personal Care Worker provides personal care services to consumers of Independence First. The Personal Care Worker is directly supervised by the Registered Nurse Care Coordinator.

Essential Functions of the Job
  1. Assisting consumers with getting in and out of bed.
  2. Assisting consumers with toileting/incontinent care.
  3. Assisting consumers with bathing or showering.
  4. Assistance with eating.
  5. Assistance with grooming, including mouth, denture, hair, shaving, and nail care.
  6. Assistance with dressing and undressing.
  7. Care of eyeglasses and hearing aids.
  8. Assistance with mobility and ambulation, including use of gait belt, walker, or cane.
  9. Simple transfers, including bed to chair or wheelchair and vice versa.
  10. Skin care, excluding wound care.
  11. Meal preparation and serving.
  12. Light cleaning in areas used during personal care activities.
  13. Changing of consumer's bed, washing bed linens, and consumer's personal clothing.
Additional Responsibilities
  1. Complete all timesheets and documentation of consumer's condition thoroughly, accurately, and promptly.
  2. Maintain confidentiality of consumer information.

Note: The above essential functions and qualifications are not all-inclusive; additional responsibilities may be assigned as necessary.

Experience and Skills:

The ideal Pool PCW will have total care experience and be comfortable assisting consumers with diverse needs. All PCWs must have adequate training in providing personal care services and in each skill they are assigned. Documentation of training or demonstrated skill is required; previous experience may substitute for formal training.

* Hands-on experience with durable medical equipment such as Hoyer lifts, gait belts, transfer boards, etc.

* Previous experience with total care consumers.

* Excellent communication skills and ability to relate well with others.

* Set availability, including weekends and evenings preferred.

* Good problem-solving skills.

* A working phone with voicemail that is checked and responded to regularly.

* Ability to pass a caregiver background check and no criminal convictions significantly related to the job.

* Ability to travel to areas of Milwaukee, Ozaukee, Waukesha, and Washington Counties.

* Dependable and reliable.
