Personal Care Specialist - Recreation Assistant

Position Summary: Performs and assists participants with Activities of Daily Living (ADLs), monitors health status, and provides a safe environment. Assists with organization and implementation of activities, meal service, and other center services. Collaborates with the interdisciplinary team and communicates participant condition and status changes.

Essential Duties and Responsibilities
  1. Deliver high-quality care as part of the interdisciplinary team, following each participant's care plan across various settings.
  2. Provide personal care assistance, including bathing, dressing, grooming, feeding, toileting, and other daily activities.
  3. Facilitate and participate in group and one-on-one activities.
  4. Assist with meal preparation, service, monitoring, and cleanup.
  5. Maintain a clean, safe, and orderly environment.
  6. Record and monitor vital signs and health changes, reporting to team members.
  7. Assist with transportation and escort services to appointments.
  8. Develop and implement individualized care plans in collaboration with the IDT.
  9. Maintain accurate documentation in medical records.
  10. Protect privacy and confidentiality of all information.
  11. Foster positive relationships with participants, families, and staff.
  12. Participate in team meetings, training, and quality improvement activities.
  13. Follow organizational policies, procedures, and OSHA safety guidelines.
  14. Perform additional duties as assigned.
Experience and Education
  • Two (2) years of experience in a hospital, nursing home, or community setting preferred.
  • License or certification as required by state.
  • Experience working with elderly or frail populations preferred; training provided if not.
Pre-employment Requirements
  • Reliable transportation, valid driver's license, and auto insurance.
  • Up-to-date immunizations and clearance for communicable diseases.
