Responsibilities

• Assist participants with Activities of Daily Living (ADLs), such as personal care, toileting, meals, and laundry.
• Monitor health status and ensure a safe environment.
• Support and participate in activities, meal services, and other center functions.
• Record vital signs and health changes, reporting to the team.
• Assist with transportation and accompany participants to appointments.
• Collaborate with the interdisciplinary team to develop and follow care plans.
• Maintain accurate documentation and uphold confidentiality.
• Build positive relationships with participants, families, and colleagues.
• Participate in team meetings, training, and ongoing education.

Qualifications

• Two years of experience in healthcare settings preferred.
• Relevant license or certification as per state requirements.
• Experience with elderly populations is preferred; training provided if not.

Pre-employment Requirements

• Reliable transportation, valid driver’s license, and insurance.
• Up-to-date immunizations and medical clearance for communicable diseases.

Work Schedule

Monday - Friday, 8am - 4:30pm, 40 hours/week.
