Personal Care Specialist

POSITION SUMMARY: Performs and assists participants with Activities of Daily Living (ADLs) such as participant personal care and toileting, assistance with meals, laundry, restorative and supportive care. Monitors health status and provides a safe environment for participants. Assists with organization and implementation of activity program, meal service, and other center services as directed. Functions as a member of the interdisciplinary team and communicates with team members regarding participant condition and status changes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Deliver high-quality care as a member of the PACE interdisciplinary team (IDT), following each participant’s plan of care across various settings, including the center, clinic, contracted facilities, and participants’ homes.
  • Provide personal care assistance, including bathing, dressing, grooming, feeding, toileting, and other daily living activities.
  • Facilitate and participate in group and one-on-one activities aligned with the daily activity calendar.
  • Assist with meal preparation, service, monitoring, and cleanup.
  • Maintain a clean, safe, and orderly environment for participants throughout the day.
  • Record and monitor vital signs, weights, and changes in participants’ physical or emotional health, reporting to the appropriate team members.
  • Accompany participants to appointments, assist with transportation, and provide escort services ensuring participant safety.
  • Collaborate with the IDT to develop and implement individualized care plans and communicate participant needs effectively.
  • Maintain accurate documentation in participant medical records and meet charting/reporting requirements.
  • Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants, and families.
  • Foster positive relationships with participants, families, co-workers, and community members.
  • Participate in team meetings, in-services, and training to support quality improvement and ongoing education.
  • Follow organizational policies, procedures, and OSHA safety guidelines, maintaining confidentiality and professional standards.
  • Participate in continuing education classes and any required staff and training meetings.
  • Perform additional duties as assigned to support the care and well-being of participants.

EXPERIENCE AND EDUCATION:

  • Two (2) years of experience working in a hospital, nursing home or community-based setting is preferable.
  • License/Certification as applicable per State requirements
  • 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
  • Position specific competencies for the Personal Care Specialist will be met prior to assuming participant care.

PRE-EMPLOYMENT REQUIREMENTS:

  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
