About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.

Position Summary:

Under the supervision of the Center Director and the Personal Care Supervisor the CNA/Personal Care Aide provides support to the team in the delivery of personal care to participants and to participants in need of companionship services to and from medical appointments. Services include participant care activities such as showering, toileting, serving, feeding and other Activities of Daily Living.


Essential Functions:

Provides direct personal care to participants as directed or assigned. Duties will include showering, toileting, laundry and feeding assistance and companionship services to and from medical appointments.
Provides support and assistance with meal and snack service.
Provides support in the kitchen as needed/assigned.
Provides support in therapy/recreation as needed/assigned.
Responsible for medical chart daily entry of Day Center activities for Participants.
Assists in maintaining patient areas in a clean and orderly manner. (Cleans and organizes program areas including cleaning of equipment and shower/bathroom areas. Disinfecting)
Assists with home delivery of meals and medical supplies as needed/assigned.
Assists with front desk phone coverage as needed/requested.
Assists other staff in carrying out program assignments.
Follows infectious disease precautions to prevent the spread of organism.
Provides assistance at SIL and participant home, assists with ADLs and IADLS, transfers, companionship to doctor's appointments, etc.
Responsible for completing all CNA/PCA required training.
Performs clerical and administrative work as required including making and answering phone calls, assisting with the implementation of I-SAT and HOS-M surveys and entering participant data in EHR system.
Communicates effectively with participants and co-workers. Works effectively in a team setting.
Participates in interdisciplinary team meetings as needed.
Adheres to GMWP attendance and punctuality policies and practices.
Attends center staff meetings and in-service educational training programs. Serves on committees as directed.
Performs other related duties as assigned or requested.
Work for this position is required on site Monday-Friday Weekend hours available for special assignments and/or home coverage.
Must be flexible and open to change. Accept assigned duties in a cooperative manner.
Will require use of personal vehicle.

Education and Certification:

Must possess, as a minimum, a GED or high school diploma prior to hire.

CPR and BLS certification required.
Current Driver license, clean driving record and proof of auto Insurance.


Skills & Experience:


Knowledge of the principles and techniques involved in relating to frail elderly persons.
Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants.
Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and responsiveness to participant requests.
Ability to follow directions, verbal and written instructions, and adapt work schedule under supervision.
Ability to follow through with instructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director.
Ability to utilize appropriate safety measures when providing care.
Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner
Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, and peers in particular.
Well organized, dependable, flexible, and resourceful.


Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
