Job summary

Dartmouth Health is seeking a Per Diem Personal Care Assistant (PCA) to provide non-medical, hands-on support to clients in their homes. Responsibilities include meal prep, light housekeeping, medication cueing, errands, and ensuring safety and independence for clients.

Per diem shifts with day hours are available; experience with elderly or disabled care is preferred. The role requires a high school diploma or equivalent and willingness to learn iPhone apps used by the organization and state

Qualifications

  • Must be willing to learn to use iPhone and apps required by VNH and the states of Vermont and New Hampshire.
  • Experience with personal care of the disabled and/or elderly preferred.
  • Ability to follow Plan of Care and maintain confidentiality.

Responsibilities

  • Provide direct care to the client under supervision as outlined in the Plan of Care.
  • Prepare meals; grocery shopping, storage and safe handling of food.
  • Clean living areas and manage household tasks as needed.
  • Complete errands for the client.
  • Provide medication cueing without administering medications.
  • Notify supervisor of changes in client condition or home environment.
  • Record activities and submit documents on time; e.g., time sheets, errand sheets.
  • Adhere to infection control policies and client rights and responsibilities.

Overview
Personal Care Assistant, Per Diem

Provides non-medical services, hands-on client contact including but not limited to: homemaking, helping with activities of daily living such as meal plan preparation, grooming, toileting, eating, dressing, reminding clients to take medications, and transportation in order to support client independence and safety so they may remain in their home.

Responsibilities
  1. Under the supervision of the Long-Term Care Services Manager the PCA provides direct care to the client as outlined in the Plan of Care.
  2. Preparing meals; grocery shopping, storage, general cooking, storing in containers, utilizing safe food handling standards.
  3. Cleaning living areas including kitchen, bathroom, bedroom and sitting areas; includes the possible use of vacuum cleaners, brooms, general household cleaners, general household trash removal.
  4. Completing errands for the client.
  5. Medication cueing without administering.
  6. Informs the Long-Term Care Services Manager of any changes in the client’s condition or home environment.
  7. Records activities and submits client-related and administrative paperwork on a timely basis, i.e., time sheets, errand sheets, or other documentation.
  8. Adheres to all policies and procedures including the infection control policy and client rights and responsibilities.
  9. Understands and maintains confidentiality.

Qualifications
REQUIRED QUALIFICATIONS
  • High school graduate or equivalent
  • Must be willing to learn to use iPhone and apps required by VNH and the states of Vermont and New Hampshire.
PREFERRED QUALIFICATIONS
  • Experience with personal care of the disabled and/or elderly preferred.
  • Ability to use iPhone preferred.
