As our agency continues to grow, we are building a specialized Client Resource Team to ensure every client receives uninterrupted, high-quality care. Our Client Care Resource Specialists play a critical role in supporting new client admissions, staffing continuity, and temporary coverage across our service areas.

Position Summary

Grace n' Care is seeking experienced, dependable, and compassionate Personal Care Assistants (PCAs) to join our Client Resource Team as Client Care Resource Specialists.

Unlike traditional PCA positions that are assigned to one permanent client, Client Care Resource Specialists provide care and support across multiple client assignments based on agency needs and employee availability. This role helps ensure continuity of care by supporting new client start-ups, staff vacations, temporary staffing shortages, schedule changes, and emergency coverage.

This position is ideal for experienced caregivers who enjoy variety, flexibility, and making a meaningful impact throughout the communities we serve.

Essential Responsibilities

As a Client Care Resource Specialist, you will:

  • Provide person-centered care in accordance with each client's Care Plan.
  • Assist clients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, hygiene, and mobility.
  • Assist with transfers, ambulation, positioning, and range‑of‑motion activities as appropriate.
  • Prepare meals and assist with feeding when needed.
  • Perform light housekeeping related to the client's care.
  • Provide companionship and emotional support.
  • Assist with community outings and appointments when authorized.
  • Observe, document, and promptly report changes in client condition.
  • Complete accurate documentation using the agency's Electronic Visit Verification (EVV) system.
  • Maintain client confidentiality, professionalism, and compliance with agency policies and Minnesota regulations.
Client Resource Team Responsibilities
  • Support multiple clients throughout the Twin Cities Metro Area.
  • Assist with new client admissions and service start-ups.
  • Provide temporary coverage for vacations, call‑offs, leaves of absence, and staffing shortages.
  • Travel between client assignments as needed.
  • Maintain open communication regarding availability.
  • Demonstrate flexibility while maintaining professionalism and reliability.
  • Represent Grace n' Care positively with clients, families, and referral partners.

Client Care Resource Specialists are not permanently assigned to one client. Assignments are based on agency needs while respecting each employee's availability whenever possible.

Required Qualifications
  • High School Diploma or GED.
  • Minimum of one (1) year of professional experience as a PCA, CNA, DSP, Home Health Aide, or in a similar direct care role.
  • Valid Driver's License.
  • Reliable transportation.
  • Ability to travel throughout the Twin Cities Metro Area.
  • Ability to pass a Minnesota DHS Background Study.
  • Strong communication and interpersonal skills.
  • Ability to work independently with minimal supervision.
  • Professional, dependable, and compassionate attitude.
Preferred Qualifications
  • Two (2) to three (3) years of direct care experience in home care, assisted living, nursing homes, hospitals, or disability services.
  • Experience working with seniors and individuals with developmental disabilities, physical disabilities, dementia, or mental health conditions.
  • CPR/First Aid Certification (or willingness to obtain).
  • Experience using Electronic Visit Verification (EVV) systems.
  • Bilingual or multilingual applicants are encouraged to apply.

  • Flexible scheduling based on employee availability and agency needs.
