TriHealth's PACE of Cincinnati in Norwood, OH is seeking a Personal Care Assistant to support older adults at the PACE Center. You will work as part of the interdisciplinary team to deliver compassionate, community-based care.

Responsibilities include personal care per individual plans, assisting with transfers, meals, and mobility; documenting care in the PACE EHR; escorting participants to appointments; and collaborating with IDT management to monitor health and well-being.

Qualifikationen

• High school diploma or GED preferred.
• Basic Life Support (BLS) certification preferred or required on hire.
• Reliable transportation to and from the PACE Center.

Aufgaben

• Provide personal care per individual care plans (peri-care, grooming, dressing, bathing, toileting, feeding, ambulation).
• Assist with transfers and mobility as directed by the IDT using mechanical aids when needed.
• Accompany participants to outside medical appointments as assigned.
• Monitor health/psychosocial status and report changes to the IDT or management.
• Attend IDT and morning meetings and complete PCA documentation in the PACE EHR.
• Support QI activities and adhere to PACE center policies.
