Maxim Healthcare is seeking a Personal Care Assistant to support clients with developmental disabilities, assisting with personal care, mobility and therapeutic needs in a home or community setting.

Qualifications

• Excellent communication abilities with clients, families and care team.
• Proficiency in English; clear documentation and reporting.
• Ability to coordinate services between home, school, local health agencies and community resources.
• Computer literacy for electronic health records and scheduling.
• Must be at least 18 years of age.

Responsibilities

• Provides services to persons who need help with day-to-day activities.
• Provides health care tasks, personal hygiene services, and other related support services to clients.
• Assists with transfers and ambulation.
• Performs incidental household services that are an integral part of a personal care plan.
• Provides companionship by reading, listening and talking with client.
• Assists client in developing independent skills through self-care activities.
• Reports changes in client status or as directed by care plan.
• Performs other duties as assigned/necessary.
