Job summary

Dartmouth Health Home Care is seeking a Patient Care Assistant in Vermont. The role involves non-medical home care, helping with daily living activities, meal prep, housekeeping, and safe transportation to support client independence in their home.

You'll work Monday-Friday, 8:00 am to 4:30 pm, with a focus on compassionate patient-centered care and teamwork within an interdisciplinary group.

Qualifications

• High school graduate or equivalent.
• Willing to learn to use iPhone and apps required by VNH and the states of Vermont and New Hampshire.
• Experience with personal care of the disabled and/or elderly preferred.
• No licensure or certification required.

Responsibilities

• Provides non-medical services and hands-on client contact including homemaking and assisting with activities of daily living to support client independence and safety at home.
• Prepare meals; grocery shopping and general cooking following safe food handling standards.
• Clean living areas (kitchen, bathroom, bedroom, sitting areas) and assist with errands.
• Medication cueing without administering.
• Inform supervisor of changes in client condition or home environment and complete time sheets and paperwork.
• Adhere to policies, including infection control and confidentiality.

Responsibilities

Provides non-medical services, hands-on client contact including but not limited to: homemaking, helping with activities of daily living such as meal plan preparation, grooming, toileting, eating, dressing, reminding clients to take medications, and transportation in order to support client independence and safety so they may remain in their home.

• Under the supervision of the Long-Term Care Services Manager the PCA provides direct care to the client as outlined in the Plan of Care.
• Preparing meals; grocery shopping, storage, general cooking, storing in containers, utilizing safe food handling standards.
• Cleaning living areas including kitchen, bathroom, bedroom and sitting areas; includes the possible use of vacuum cleaners, brooms, general household cleaners, general household trash removal.
• Completing errands for the client.
• Medication cueing without administering.
• Informs the Long-Term Care Services Manager of any changes in the client's condition or home environment.
• Records activities and submits client-related and administrative paperwork on a timely basis, i.e., time sheets, errand sheets, or other documentation.
• Adheres to all policies and procedures including the infection control policy and client rights and responsibilities.
• Understands and maintains confidentiality.
