Location: Revere PACE - 10 Garofalo Street
Position Summary: Assists PACE Center participants with personal care, rehabilitation, meals, activities, and transportation. Supports participant care planning, reports to staff, and may support other sites as needed.

Responsibilities include:

• Greeting participants, assisting with daily activities, and supervising meal services.
• Attending daily reports and reporting concerns.
• Preparing participants for departure and escorting them during transport.
• Assisting with ADLs and accompanying participants to appointments.
• Carrying out treatments under nursing supervision and supporting recreational activities.
• Documenting activities, maintaining supplies, and contributing to program improvements.
• Supporting food safety and adhering to dietary restrictions.
• Implementing restorative therapy and safe transfer techniques.
• Providing ADL assistance, ensuring safety, and reporting equipment issues.
• Supporting oxygen needs and maintaining a clean environment.

Hourly rate: $21.00 - $30.00 based on experience.
