Summary of Responsibilities: The Personal Care Assistant provides quality direct and indirect care and assistance for residents in Personal Care and Memory Support. This position is expected to support all areas within Personal Care.

Responsibilities
  • Assists with activities of daily living (ADLs and IADLs) including transfers, mobility, dressing, personal hygiene (including mouth care, foot care, showering), episodes of incontinence, shaving, medication storage and distribution, ambulation, transportation coordination and companionship, light housekeeping, and the dining experience. Monitors oxygen and performs glucose monitoring.
  • Monitors and reports changes in resident conditions and reports to licensed nurse or administrator.
  • Assists residents with medications and treatments prescribed for self-administration under the direction of a licensed nurse.
  • Communicates effectively with families, PCPs and pharmacies as needed.
  • Serves meals and provides cues and assistance as needed to promote an engaged dining experience.
  • Performs light housekeeping duties in resident apartments and common areas (trash collection, linen changes and bed making, light vacuuming, dusting, and spot cleaning).
  • Engages in the Hearthstone Philosophy by following the daily flow and participates in programming with residents (small groups) such as cooking, baking, crafts, music and social activities.
  • Documents accurately in Point Click Care and maintains open communication during shift and shift change.
  • Learns life stories and engages residents based on preferences; encourages independence in daily living activities where appropriate.
  • Understands the special needs of cognitively impaired residents and utilizes appropriate interventions.
  • Observes and reports changes in condition or behavior to supervisor promptly; remains flexible and adapts to changing situations.
  • Follows THAW policies and procedures; practices infection control and safety awareness; participates in resident safety improvements.
  • Reports unsafe equipment and hazards to supervisor promptly; has knowledge of DHS regulations as they pertain to daily assignments.
  • Participates in and completes all required ongoing training (monthly online and in-person training, annual skills competencies, and role-specific certifications).

Position Type / Expected Hours
  • Ability to work nonstandard hours including evenings, weekends, holidays, and during inclement weather.
Hours and Benefits
  • Per-diem position with no guaranteed hours or shift assignment.
  • Not benefits eligible.
  • Expected to work across all levels of care at The Highlands at Wyomissing.
Education and Experience Requirements
  • High school diploma or GED (Required).
  • 1-3 years nurse aide experience with older adults in a long-term care or dementia facility (Preferred).
  • Temple University Non-Credit Direct Care Staff Training Certification.
  • DHS Medication Certification or ability to obtain within 30 days of employment and maintain compliance.
  • Diabetic Certification.
  • CPR/First Aid Certification.
  • PA Nurse Aide Certification (preferred).
  • Completion of Highlands Hearthstone Ambassador training within 90 days of employment (Required).
Work Authorization / Security Clearances
  • Medical Screening; Criminal History Clearance; FBI Fingerprint Clearance (if not in PA for last 2 years); Immigration and Naturalization (I-9) Clearance.
Physical Requirements
  • Hearing, speaking English clearly, writing/typing for documentation, and reading printed/computer texts.
  • Physical demands: standing, walking, bending, lifting (up to 50 lbs occasionally) with assistance; ability to push/pull and lift as required.
Safety Hazards and Environment
  • May perform emergency care; exposure to bloodborne pathogens, communicable diseases, high noise levels, moving parts, electrical hazards, chemicals, and rare aggressive interactions.
