Job summary

PsychPros, Inc. is seeking a Housing Case Manager in Altadena, CA for a full-time temporary assignment.

You will provide intensive case management to clients in a permanent supportive housing program, assisting with intake, housing access, lease-up, and coordination of health services. The role requires 2 years of human services experience or a bachelor's degree in Social Work or related field, and a valid CA driver’s license with reliable access to a registered vehicle.

Responsibilities

• Provide intensive case management to clients living in a permanent supportive housing program who also have a chronic health condition.
• Conduct intake and enrollment with eligible clients.
• Assist clients with accessing temporary housing until permanent housing is secured.
• Support clients with the lease-up process.
• Conduct comprehensive bio-psycho-social assessments.
• Develop individualized collaborative service plans.
• Assist clients with their physical and mental health needs by providing support and linkage to appropriate services.
• Transport clients as needed to essential appointments.

PsychPros is a nationally recognized staffing and recruiting firm dedicated to the behavioral healthcare field. We are currently hiring a Housing Case Manager to work full-time with clients experiencing homelessness in Altadena, CA. This is a TEMPORARY ASSIGNMENT with no set end date.

Requirements

• 2 years of experience in the human services field OR a bachelor’s degree in Social Work or in a related field
• Possess a valid California driver's license and have access to properly registered vehicle

Pay

• $23.50/hour

Schedule

• Monday-Friday 8:30am-5pm
