Job summary

Talented-Medical-Solutions is seeking a Speech-Language Pathologist (SLP) for a permanent position at a 126-bed skilled nursing facility in Fremont, California. New graduates are welcome to apply.

Qualifications

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current California Speech-Language Pathologist license or eligibility for California licensure.
  • CCC from ASHA or eligibility preferred.
  • Strong clinical, communication, and organizational skills.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Compassionate, patient-focused approach to care.

Responsibilities

  • Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
  • Develop individualized treatment plans based on residents' clinical needs and goals.
  • Provide skilled speech-language therapy using evidence-based practices.
  • Monitor patient progress and adjust treatment plans as appropriate.
  • Evaluate and treat communication, cognition, and swallowing disorders.
  • Complete accurate and timely clinical documentation.
  • Participate in interdisciplinary care planning and collaborate with nursing and other rehabilitation professionals.
  • Educate residents, families, and caregivers on treatment strategies and recommendations.
  • Communicate effectively with the care team regarding patient progress and changes in condition.
  • Promote patient safety, independence, and quality of life through individualized care.
