Job summary

SCO Family of Services is seeking a Case Planner for Enhanced Family Foster Care. Location includes Queens, Brooklyn, and Long Island. You will support safety, permanency, and well-being by developing and monitoring service plans with families, addressing barriers, and coordinating referrals.

Requirements include a Bachelor’s degree (BSW/MSW preferred), strong communication, proficiency in MS Office and Connections, and a NYS driver license. Bilingual abilities are encouraged.

Qualifications

• Bachelor's degree required; BSW or MSW preferred.
• Excellent oral and written communication skills.
• Proficiency in Microsoft Office, especially Excel; familiarity with Connections.
• Bilingual abilities are strongly encouraged.
• Valid NYS Driver License required.

Responsibilities

• Complete initial face-to-face contact with the child in the foster home within 2 business days.
• Maintain ongoing face-to-face contacts with child, parent, and foster parent to assess safety and risk.
• Complete safety checklists and risk questionnaires during casework contacts monthly.
• Count and monitor medication and medication compliance.
• Complete timely documentation in Connections within 7 business days of events.
