Job summary

Department of Human Services in Oregon seeks a Permanency Worker to engage with families and build connections that support safety, stability, and permanency for children in care.

The role requires a driver’s license and a relevant degree with experience in human services or case management. You will coordinate services, monitor progress, and collaborate with partners to achieve best outcomes for children.

Qualifications

• Valid driver's license required.
• Bachelor's in Human Services or related field or equivalent experience.
• Experience in case management or social services preferred.

Responsibilities

• Monitor safety, well-being, and development of children in care; support transitions.
• Facilitate and observe parent-child visits; provide coaching and transport arrangements.
• Maintain contact with families and partners; update safety and service plans.
• Coordinate services, make referrals, and track progress for children and families.
• Maintain accurate case records, including assessments, plans, and timelines.
• Prepare reports, make recommendations, participate in hearings and reviews.
• Collaborate with caregivers, schools, providers, and DHS partners.
