Job summary

Qualifications

• Master's Degree in Social Work from an accredited school.
• New York State Registration as a Licensed Clinical Social Worker (LCSW) and at least one year of post-graduate experience in family conferencing or related clinical work.
• New York State Registration as a Licensed Master of Social Work (LMSW) with at least three years of relevant experience; up to two years may be in supervision; or LCSW/LMSW must be obtained within one year of appointment.

Responsibilities

• Attend and facilitate all Family Team Conferences focusing on child safety, risk and well-being, permanency, and ensure placement and discharge decisions reflect ACS commitments.
• Address concerns of the child, family, provider agencies, and court during conferences.
• Prepare and complete administrative functions including documentation of conference, data, and action plans.
• Complete Expedited Home study packages with kinship resources in the community.
• Interface with interagency support resources (clinical consultation team, etc.) to identify issues and barriers.
• Provide conferencing and consultation to reassess decisions and service plans; advise on legal posture and court orders.
