CASE MANAGER SPECIFIC DUTIES:Learn, understand, and implement policies and procedures regarding usage of the CANS/FAST, Casey Life Skills tools used in our Preparation for Independent Living Program, Ages and Stages assessments and our Evolv software program.Write letters, progress reports, Individual Service Plans, Individual Service Plan Amendments, incident reports (HCSIS), and other written reports as required.Meet with families and children in care and monitor progress in placements. Participate in and provide leadership for planning and implementing an excellent standard of care for children, including an appropriate provision of room, board, supervision, clothing, religious opportunities, social opportunities, cultural opportunities, independent living preparation, education, and medical and dental care.Assure that transportation is available as needed, and safely transport children and families when appropriate. Assure company and personal cars are safe and in good condition.Recruit resource families and conduct family studies and reevaluations.Assure resource families meet agency requirements for training, plan with and assist families in training and personal development, which goes beyond minimum requirements.Be closely and continually involved with children, birth families and resource families regarding solving interpersonal conflicts, and developing and maintaining relationships.Plan with all children, resource families, birth families and referring agencies regarding visits, reunification and/or other possible permanency goals for the future. Monitor and supervise the process as necessary to assure goals are met.Participate in the process of accepting children referred for care in a manner which insures FCCY meets the responsibility of placing the needs, rights, and welfare of children and families as the first priority. Attend meetings and conferences, gather information, arrange and supervise visits and perform any necessary function to accomplish timely and appropriate placement.The Permanency Case Manager is to be able to:Make plans and collateral contacts.Make arrangements for or help Resource parents make arrangements for clients.Provide direct help to assure practical problems are solvedEnsure children in placement receive medical care, dental care, cultural opportunities, education, religious opportunities, supervision, and clothing.Ensure family visits are arranged for and followed through.Ensure clients are prepared for independent living,Ensure clients receive recreational and social opportunities.Be prepared to share an on-call schedule with other co-workers and handle all calls according to agency policy and procedure.Manage a caseload, which averages twelve children over a year’s time. This means during the course of a year a case load can go up or down as circumstances change on a regular basis.The Permanency Case Manager is responsible for helping clients move to a permanent living arrangement as soon as possible.The Permanency Case Manager may be requested to assist with adoption duties and functions such as:Child PreparationFamily ProfileChild ProfileFinalizationsChild Specific Recruitment.GENERAL DUTIES:Learn, understand, accept, and implement agency policy, and applicable laws and regulations.Attend required meetings, conferences, trainings, and other agency functions, manage work time appropriately on a forty-hour per week flex time schedule, be punctual for appointments, and complete assignments and reports on time.Communicate fully, honestly and clearly in an appropriate positive manner, which furthers the agency’s purpose and promotes cooperation and teamwork. Keep supervisor, administration and referring agency fully informed, in a timely manner, of important issues.Assist and support resource families, and children placed with them, to develop relationships, which will make possible and promote a safe and welcoming home. Participate in and provide leadership in planning and carrying out activities, which will promote well-being, happiness, and ability to solve problems and personal development for children.Look ahead, reason, anticipate, inform appropriate people and act to better prevent problems from occurring and encourage positive opportunities for children to develop.Prepare written plans and carry out planning processes as required by agency policy and regulations.Maintain complete, clear, comprehensive, neat, organized records for children and families in compliance with policy and regulation. (NOTE: This is a Permanency Case Manager responsibility and not clerical staff responsibility – clerical staff creates the files and files).Keep desk and work area clean, neat and in order. Avoid giving messages of anger, defiance of rule, law or generally accepted morality, excessive pressure or disorganization. Give positive messages, which further the agency’s purpose in non-verbal communication including dress, manner and condition of work environment. Understand the difference and give positive rather than negative harmful messages by use of good taste and judgment.Relate to colleagues, Resource Parent, and other professionals in a manner, which promotes cooperation and teamwork. Ensure communication demonstrates courtesy, respect, appropriate concern, friendliness, and mature control of feelings and self in all interactions.Make appropriate written suggestions for improvement in policies.REQUIREMENTS:The Permanency Case Manager position requires a bachelor’s degree in a Human Services field, current Pennsylvania driver’s license, and eligibility under current Pennsylvania legal and regulatory requirements. In addition, this position requires the employee to be fully vaccinated for Covid-19. A Permanency Case Manager must demonstrate an ability to understand, accept, and implement agency philosophy and policies: an ability to form appropriate relationships to work successfully with adults and children from a variety of backgrounds; to promote, encourage and participate in teamwork efforts; and have an ability to solve problems in a resourceful and appropriate way.
